First, see How to Unlock Chapter 5 Secret Area. On the way to the secret area you will reach the Cooling Slope shrine automatically, it's directly in front of the secret area entrance. From the Cooling Slope shrine, go through the frozen gate to the secret area (only possible after unlocking secret area), then in the big round area you will find this frog boss on the left side. This is BEFORE actually entering the "Bishui Cave" secret area. Alternatively, after beating the Chapter 5 Secret Area boss "Bishui Golden-Eyed Beast", you will be teleported back outside and then the frog boss will be to your right.
