For this questline you must find 3 sword shards (Forge it Anew: Do Not Disturb, Forge it Anew: Hostile Takeover, Forge it Anew: Executive Order). Then you must also find St. Mihai's Sword (Side Quests: Night Terrors, A Monk and a Saint). Then bring all pieces to the Uriashi Blacksmith in the north-west of Briar Sloughs region. She will craft it into the finished sword and tells you to go to a nearby cave to perform a ritual to temper the sword. This turns it into the fully restored St. Mihai's sword (one of the best legendary weapons in the game) and unlocks the Arbiter's Sword trophy / achievement.
